Field of Eden Senior Living – Visitor Log Book Disclaimer

The Visitor Log Book is maintained to ensure the safety, privacy, and well-being of all residents, staff, and visitors of Field of Eden Senior Living. By signing this log, you acknowledge and agree to the following terms and conditions.

1. Visitor Identification and Entry
   All visitors must sign in and out at the designated area upon arrival and departure.
    A valid photo ID may be requested for verification.
    Visitors must wear a visitor badge if issued and remain in approved areas only.
     Entry is subject to staff approval and may be denied for safety, health, or policy reasons.

2. Health and Safety Compliance
     Visitors agree to follow all infection control policies, including proper hand hygiene and use of masks when required.
     Visitors with symptoms of illness (fever, cough, sore throat, contagious condition, etc.) may not enter the facility.
     Emergency or evacuation procedures must be followed at all times as instructed by staff.

3. Resident Privacy and Confidentiality
     Visitors are prohibited from discussing or disclosing any resident information outside the facility.
     No photos, videos, or audio recordings of residents, staff, or facility operations are allowed without written consent (LIC 627).
     Visitors must respect residents’ rights to dignity, privacy, and personal space at all times.

4. Conduct and Behavior
    Respectful and calm behavior is expected at all times.
    Abusive language, harassment, or disruptive conduct will result in immediate removal from the premises.

    Smoking, alcohol, or drug use on the property is strictly prohibited.
    Children must be supervised by an adult visitor at all times.

5. Property and Safety Visitors must not tamper with equipment, furniture, or resident belongings.
   Any accident or unusual incident must be reported immediately to staff.
   The facility is not responsible for lost or stolen personal items.

6. Visitation Hours and Special Circumstances
    Standard visiting hours are 9:00 AM to 5:00 PM, unless special arrangements are approved by the administrator.
    After-hours visits must be pre-approved and recorded by staff.
   During outbreaks, emergencies, or special restrictions, visitation may be limited per public health or community care licensing guidelines.
